Bhavaprakasha Nighantu, Kshudra Dravya Varga
Kumari pittasraja dosha tridoshaghni — Kumari destroys Pitta, Rakta disorders, and is tridoshaghna (balancing to all three Dosha when used correctly). The fresh gel is cooling and liver-protective; the bitter resin is purgative and emmenagogue. These two preparations are opposite in action and must not be confused.

The two preparations — critical distinction

Kumari Svarasa (inner gel juice): Cooling, Pitta-reducing, liver-protective, demulcent. Indicated for: Yakritvikar (liver conditions), Pittaja skin conditions with burning, Artava Kshaya (delayed or scanty menstruation — as a gentle uterine tonic), gastric inflammation. The commercial Aloe vera juice from inner fillet processing corresponds to this preparation.

Kumari Sara (outer leaf latex — Musabbar): The bitter yellow-brown latex containing aloin. Strongly purgative — classical Rechana (purgative) action. Emmenagogue — stimulates uterine contractions. Contraindicated in pregnancy. This preparation is the 'Aloe' used in Western herbal medicine for constipation and corresponds to the purified extract 'Aloe-emodin' in pharmaceutical preparations.

Acemannan and modern research
The primary polysaccharide in Aloe vera inner gel — Acemannan (acetylated polymannose) — has documented immunomodulatory, wound-healing, and gastroprotective properties. Research in the Journal of Ethnopharmacology and Phytomedicine documents Aloe vera gel for gastric ulcer protection (consistent with classical Pittaja gastric indications), wound healing (classical Vrana management use), and hepatoprotective effects (classical Yakritvikar use). Pregnancy contraindication
Kumari Sara (aloe latex) is absolutely contraindicated in pregnancy — the anthraquinone compounds stimulate uterine contractions. Even commercial Aloe vera juice products that are not properly filtered to remove latex components should be avoided in pregnancy. The inner fillet gel without latex is considered safer but classical texts advise caution throughout pregnancy with any Artavajanan (emmenagogue) herb.
