Shallaki shotha shamanam... sandhiposhanam -- Shallaki reduces swelling (Shotha), relieves pain, and nourishes the joints (Sandhiposhanam). It is the primary classical preparation for conditions of Sandhi (joint) inflammation.

Classical documentation

Charaka Samhita documents Shallaki in the Shothaghna (anti-oedema/anti-inflammatory) and Vedanasthapana (analgesic) groups. Sushruta Samhita documents the resin in wound healing (Vrana) preparations. Classical compound preparations: Shallaki Guggulu (with Guggulu resin -- combining the anti-inflammatory Boswellia with Guggulu's Lekhana action for joint conditions); Shallaki Taila (external oil for joint massage); and compound Churna preparations for musculoskeletal Vata conditions.

AKBA and clinical trial evidence
Boswellic acids -- particularly AKBA (3-O-Acetyl-11-keto-beta-boswellic acid) -- are the primary anti-inflammatory compounds, documented as inhibiting 5-lipoxygenase (5-LOX), the enzyme that produces pro-inflammatory leukotrienes. Multiple double-blind RCTs have documented Boswellia serrata extract for knee osteoarthritis with significant pain reduction and functional improvement. A meta-analysis published in PLOS ONE (2014) covering multiple trials confirms significant benefit over placebo. The European Medicines Agency (EMA) has granted 'well-established use' status to Boswellia serrata preparations for osteoarthritis. This is one of the strongest clinical trial evidence bases among all documented Ayurvedic herbs.
Standardisation note
Commercial Shallaki preparations vary enormously in boswellic acid content. API specifies minimum boswellic acid content. The pharmaceutical-grade Boswellia extracts used in clinical trials (typically standardised to 65--85% boswellic acids including minimum AKBA percentage) are distinct from unprocessed resin or low-standardisation commercial preparations. For clinical use, standardised extract preparations from API-compliant manufacturers are the quality standard.
