Charaka Samhita, Chikitsasthana 26.3
Mutrakrichre vata pitta kapha tridosha sannipata raktaja — Mutra Krichra arises from Vata, Pitta, Kapha, their combinations, and Rakta (blood-related). Eight types are documented. The treatment varies entirely by type — cooling treatments for Pittaja types will aggravate Vataja types.

The eight classical types and their presentations

Vataja: Scanty, irregular urination with pain; lower abdominal distension. Pittaja: Burning urination, yellow or reddish urine, associated fever — corresponds to UTI type. Kaphaja: Mucus in urine, difficulty initiating urination, heaviness. Sannipataja: Features of all three. Raktaja: Blood in urine (Haematuria). Sarkara: Sandy/gritty deposits in urine — early stage calculi. Mutrashmari: Formed urinary stones. Shukraja: Semen-related urinary obstruction.

Mutrashmari — urinary stones

Charaka Samhita and Sushruta Samhita both document Mutrashmari (urinary calculi) separately. Four classical stone types based on composition: Vataja (rough, irregular), Pittaja (dark, with burning), Kaphaja (smooth, round, pale), and Shukraja (seminal calculi). Classical management: Gokshura (Ashmarighna — stone-dissolving property documented in API and supported by research on Tribulus terrestris reducing calcium oxalate crystal formation); Pashanabheda (Bergenia ligulata — the 'stone-breaker' herb); Punarnava (diuretic, reduces urinary stasis).

Primary classical herbs
Gokshura (Tribulus terrestris) — the primary Mutrala (diuretic) and Ashmarighna (stone-dissolving) herb; API Gokshura monograph documents standards for Tribulus terrestris; Gokshuradi Guggulu is the primary compound formulation. Punarnava — Mutrala and Shothahara; for Kaphaja types with oedema. Chandraprabha Vati — the classical compound Vati formulation documented specifically for urinary conditions across all types.
Panchakarma in urinary conditions
Basti (therapeutic enema) is the classical Panchakarma procedure for Vataja Mutra Krichra — it acts on the Vata in the colon that governs Apana Vata (the downward-moving force governing urination and defecation). Medicated Basti preparations with Dashamoola and Gokshura are documented for Mutra Krichra conditions of Vata origin.
